Understanding Tendonitis: Why “Walking It Off” Makes Foot and Ankle Pain Worse

The 72-Hour Rule: Muscles vs. Tendons

The easiest way to diagnose your pain at home is to look at the timeline.

  • Muscles Recover Quickly.
    • Normal Delayed Onset Muscle Soreness (DOMS) usually peaks at 24 to 48 hours and fades rapidly.
    • A sore calf muscle will feel better after a light stretch.
  • Tendons Do Not.
    •  Tendons are the thick, fibrous cords that attach your muscles to your bones.
    • In the foot and ankle, overuse commonly affects the Achilles tendon, the posterior tibial tendon, or the Peroneal tendons.
    • Because they have a notoriously poor blood supply, they heal slowly.
    •  If your pain lasts longer than 72 hours, or if it feels sharp and localized, you are likely dealing with micro-tears and inflammation in the tendon (the exact definition of Tendonitis).

The Red Flags of Tendonitis

Whether the pain is in the back of your heel or running along the side of your ankle, check for these specific warning signs:

  1. The “Morning Creak”: Tendonitis is distinct because it stiffens significantly when you rest. The pain is usually sharpest during your first steps out of bed or when standing up after a long car ride.
  2. Pinpoint Pain & Bumps: General soreness covers a wide area of the leg. Tendonitis usually features “point tenderness”: you can press on one specific spot on the tendon and feel sharp pain. You may even feel a thickened or “bumpy” ridge along the tendon cord.
  3. Swelling, Warmth, and Redness: The affected tendon may look visibly thicker than the one on your other leg and radiate heat due to localized inflammation.

The Danger of “Playing Through It”

Tendonitis is a repetitive strain injury.

  • If you continue to load an inflamed tendon without proper support, you trigger Tendinosis, a state where the collagen fibers begin to degenerate and fray.
  • This drastically increases your risk of a catastrophic rupture (a completely torn tendon), which requires surgical repair and months of grueling rehab.
